ABSTRACT

Alrededor del 80 % de los casos sintomáticos de COVID-19 tienen una enfermedad leve a moderada, que no suele progresar a fases más avanzadas. El 14 % de los casos pueden progresar en unos 7 a 10 días a un cuadro severo pulmonar, mientras que un 6 % siguen deteriorándose en el tiempo ante una respuesta hiperinflamatoria o de tormenta de citoquinas, que conlleva a shock y falla de múltiples órganos. En general tienen mayor riesgo de progresión los individuos con factores de riesgo como edad mayor de 60 años, género masculino, obesidad, diabetes, hipertensión, inmunosupresión, trasplante de órganos sólidos, enfermedad renal, tabaquismo; pero eso no descarta la posibilidad aislada que individuos aparentemente sanos puedan presentar una evolución severa o diversas complicaciones pulmonares, renales, cardiovasculares, neurológicas, endocrinológicas, entre otras. Este consenso busca orientar al personal de salud en Venezuela en el abordaje terapéutico y la atención de las personas con COVID-19, estableciendo recomendaciones con base a la mejor evidencia para la fecha. Las recomendaciones no solo se limitan a definir qué opciones terapéuticas han mostrado mayor eficacia y seguridad, sino que determina cuáles drogas carecen todavía de suficiente evidencia, y qué alternativas no deberían utilizarse por carecer de beneficios y/o de seguridad establecida. La medicina basada en la evidencia busca fundamentar las decisiones clínicas con base en evidencias; que son todos los elementos y hechos que demuestran jerárquicamente el nivel de veracidad y validez de diversos planteamientos en medicina. El mayor nivel de evidencia terapéutica se construye por medio de metaanálisis y revisiones sistemáticas de la literatura con base en estudios clínicos controlados, prospectivos, con asignación al azar por doble ciego, y con una muestra lo suficientemente importante; y es este tipo de evidencia la que se ha considerado más relevante para establecer las recomendaciones.


About 80 % of symptomatic COVID-19 cases have mild to moderate illness, which does not usually progress to more advanced stages. 14 % of cases can progress in about 7 to 10 days to a severe pulmonary condition, while 6 % continue to deteriorate over time in the face of a hyperinflammatory response or cytokine storm, which leads to shock and failure of multiple organs. In general, individuals with risk factors such as age over 60 years, male gender, obesity, diabetes, hypertension, immunosuppression, solid organ transplantation, kidney disease, smoking, generally have a higher risk of progression. but that does not rule out the isolated possibility that apparently healthy individuals may present a severe evolution or various pulmonary, renal, cardiovascular, neurological, endocrinological complications, among others. This consensus seeks to guide health personnel in Venezuela in the therapeutic approach and care of people with COVID-19, establishing recommendations based on the best evidence to date. The recommendations are not only limited to defining which therapeutic options have shown greater efficacy and safety, but also determine which drugs still lack sufficient evidence, and which alternatives should not be used due to lack of benefits and / or established safety. Evidence-based medicine seeks to base evidencebased clinical decisions; which are all the elements and facts that hierarchically demonstrate the level of veracity and validity of various approaches in medicine. The highest level of therapeutic evidence is constructed through meta-analysis and systematic reviews of the literature based on controlled, prospective clinical studies, with double-blind randomization, and with a sufficiently large sample; and it is this type of evidence that has been considered most relevant to establish the recommendations.

La etiología que conduce al daño neonatal es multifactorial, y los procesos infecciosos pueden estar implicados en él. El objetivo de este estudio fue identificar microorganismos del tracto genital materno asociados con el daño neonatal, a fin de prevenir futuras complicaciones perinatológicas. Se estudiaron 711 embarazadas que concurrieron entre enero de 2010 y julio 2013 al consultorio externo de Obstetricia del Hospital de Clínicas de la UBA para sus controles prenatales, y cuyos partos también tuvieron lugar en dicho nosocomio. En la sangre del cordón umbilical se investigó la presencia de Ureaplasma urealyticum y Mycoplasma hominis mediante el cultivo con sustratos metabólicos (Micofast-Biomerieux), y la de Trichomonas vaginalis por PCR, con primers específicos. El estudio microbiológico del contenido vaginal se efectuó en 288 de las embarazadas en la semana 35 a 37. Se empleó la metodología convencional, a la que se agregó el cultivo en tioglicolato modificado para T. vaginalis. Se investigó la presencia de estreptococos grupo B (EGB) en hisopado anorrectaly de introito vaginal, utilizando enriquecimiento en caldo selectivo y posterior siembra en medio cromogénico. Se utilizaron los test de χ² Yates y de Fisher para muestras independientes, considerándose significativo p < 0,05. La vaginosis bacteriana (VB) se relacionó significativamente con el daño neonatal (p = 0,02), al igual que la presencia de M. hominis (p = 0,03) y de T. vaginalis (p = 0,03) en la sangre del cordón umbilical. Las complicaciones predominantes fueron el parto pretérmino, la rotura prematura de membrana (RPM), el bajo peso y un valor de Apgar <7. No se asoció al daño neonatal la presencia de U. urealyticum (p = 0,35) en el cordón umbilical, ni la de Candidaspp. (p = 0,94) o EGB (p = 0,18) en el tracto genital de las madres. Dado que ciertas alteraciones en la microbiota del tracto genital materno se relacionaron con el dano neonatal, consideramos de fundamental importancia realizar el estudio microbiológico del contenido vaginal durante el embarazo, para prevenir posibles complicaciones maternas y perinatológicas.


The etiology leading to neonatal damage is multifactorial, being genital infections one of the causes. The objective of the study was to identify microorganisms of the maternal genital tract that are associated with neonatal damage, in order to prevent future perinatal complications. Seven hundred and eleven pregnant patients attended their prenatal control during the period January 2010-July 2013. Ureaplasma urealyticum and Mycoplasma hominis presence was investigated in umbilical cord blood by metabolic substrates (Micofast-Biomerieux) and that of T. vaginalis, by PCR using specific primers. The microbiological study of the vaginal contents of 288 pregnant patients at weeks 35 to 37 was performed by conventional methods, adding the modified thioglycolate culture for T. vaginalis. Group B streptococcus (GBS) was investigated in anorectal and vaginal introitus swabs, using selective broth enrichment and subsequent isolation in chromogenic medium. The χ² Yates test and Fisher's test were used for independent samples. A p value <0.05 was considered statistically significant. The pathogens significantly related to neonatal damage were M. hominis (p = 0.03), T. vaginalis (p = 0.03), and BV (p = 0.02). Main complications were preterm birth, premature rupture of membranes (PRM), low weight and Apgar score <7. U. urealyticum (p = 0.35), Candidaspp. (p = 0.94) and GBS (p = 0.18) were not related to neonatal damage. Since different microorganisms of the maternal genital tract were related to neonatal damage, it is very important to perform the microbiological study of vaginal contents during pregnancy to prevent possible maternal and perinatal complications.

ABSTRACT

La sarcoidosis es una enfermedad granulomatosa no caseificante, multisistémica, de causa desconocida, que compromete al pulmón y a los ganglios linfáticos mediastinales entre el 90 y el 95% de los casos. También puede afectar otros órganos, como las glándulas salivales, piel, ojos, hígado, bazo, corazón, huesos y sistema nervioso central. La sarcoidosis tiene una baja prevalencia en Latinoamérica y es subdiagnosticada debido a la alta frecuencia de otros trastornos similares, como tuberculosis, lepra y micosis profundas. El diagnóstico presuntivo se establece con hallazgos imagenológicos característicos dentro de un contexto clínico apropiado y se confirma con la evidencia histológica de granulomas no caseificantes de células epiteliales, en ausencia de otras etiologías. Los hallazgos torácicos incluyen la afectación pulmonar, ganglionar y bronquial, los cuales son detectados a través de la radiografía (Rx) y tomografía computada (TC) de tórax, siendo esa última más sensible y específica. En este artículo, resaltamos la importancia de reconocer los patrones de presentación típicos y atípicos de la sarcoidosis en Rx y TC, así como la relevancia de las imágenes torácicas como elemento clave en el algoritmo diagnóstico de esa patología. También describimos la utilidad de la resonancia magnética (RM), como método adicional para el diagnóstico en casos de afectación cardíaca y el papel de la tomografía por emisión de positrones (PET-CT) en el seguimiento terapéutico.


Sarcoidosis is a non-caseating granulomatous, multisystemic disease of unknown cause that involves the lung and mediastinal lymph nodes in 90-95% of cases. It can also affect other organs such as the salivary glands, skin, eyes, liver, spleen, heart, bones and the central nervous system. Sarcoidosis has a low prevalence in Latin America and it is underdiagnosed due to the high frequency of other similar disorders such as tuberculosis, leprosy and deep mycosis. The presumptive diagnosis is established based on characteristic imaging findings within an appropriate clinical setting and is confirmed by histological evidence of non-caseating epithelioid cell granulomas, in the absence of other etiologies. Thoracic imaging findings include pulmonary, nodal and bronchial involvement, which are detected on chest radiography (CXR) and computed tomography (CT), this last one having a higher sensitivity and specificity. In this article, we highlight the importance of recognizing the typical and atypical presentation patterns of sarcoidosis on CXR and CT, as well as the relevance of thoracic images as key elements in the diagnostic algorithm of this pathology. We also describe the usefulness of magnetic resonance (MR) imaging as an additional method for diagnosis in cases of cardiac involvement and the role of positron emission tomography (PET-CT) in therapeutic follow-up.

Introducción: La vigilancia permanente de la calidad constituye un requerimiento ético y un compromiso de las universidades actuales, como garantía de la formación de profesionales con elevada sensibilidad social. Objetivo: Desarrollar un modelo de gestión de calidad en la educación a distancia, idóneo para la maestría de Gerencia en Salud para el Desarrollo Local, basado en las necesidades detectadas por los estudiantes y en las experiencias de buenas prácticas en esta modalidad. Métodos: Se realizó una investigación acción, con adaptación parcial de la metodología del marco lógico, en un posgrado administrado en la modalidad a distancia en la Universidad Técnica Particular de Loja, Ecuador. La experiencia se desarrolló mediante un diagnóstico de las debilidades académicas y administrativas de la maestría, a partir del cual se diseñó, implementó y evaluó un modelo de gestión de calidad, basado en las necesidades detectadas por los estudiantes, y en la revisión de estándares y experiencias de calidad. Asimismo, con un nivel de confianza de 95 por ciento, una precisión de 3 por ciento y una proporción de 5 por ciento, se calculó el tamaño muestral de 108 informantes, seleccionados al azar de una población de 230 estudiantes. Resultados: Los resultados indicaron que todos los elementos incluidos en el modelo fueron útiles para mejorar la calidad del programa. Sin embargo, se necesita continuar fortaleciendo la calidad de atención tutorial, especialmente en cuanto a los tiempos de respuesta, la afectividad de los mensajes y la empatía demostrada en los procesos de realimentación. Conclusiones: Es prioritario mejorar los aspectos administrativos implicados en las actividades presenciales, así como dar seguimiento continuo a los elementos del modelo mediante la ampliación de otros aspectos considerados en las teorías educativas y en los estándares de calidad nacional e internacional(AU)


Introduction: Permanent quality monitoring constitutes an ethical requirement and a commitment of universities nowadays, as a guarantee for the training of professionals with high social sensitivity. Objective: To develop a quality management model in distance education, suitable for the master's degree course in Health Management for Local Development, based on the needs detected by the students and experiences of good practices in this modality. Methods: An action research was carried out, with partial adaptation of the logical framework methodology, in a postgraduate course administered in the distance modality at Particular Technical University of Loja, Ecuador. The experience was developed through diagnosis of the academic and administrative weaknesses of the master's degree course, from which a quality management model was designed, implemented and evaluated, based on the needs detected by the students as well as the revision of quality standards and experiences. Likewise, with a confidence level of 95 percent, an accuracy of 3 percent, and a proportion of 5 percent, we calculated the sample size of 108 informants, randomly selected from a population of 230 students. Results: The results indicated that all the elements included in the model were useful to improve the quality of the program. However, it is necessary to continue strengthening the quality of tutorial attention, especially in terms of response times, the affectivity of messages, and the empathy demonstrated in the feedback processes. Conclusions: It is a priority to improve the administrative aspects involved in face-to-face activities, as well as to monitor continuously the elements of the model by expanding other aspects considered in educational theories and in national and international quality standards(AU)

Introducción: el toxoide pertúsico es una proteína muy utilizada en las vacunas acelulares contra la tosferina. Desarrollar un protocolo para su purificación con pocos pasos y con parámetros de purificación adecuados, es muy importante para su caracterización química y la valoración de los efectos adversos que puede provocar al inocular principalmente a ratones. Los protocolos de purificación descritos en la literatura científica involucran varios pasos de purificación y bajos rendimientos. Objetivo: desarrollar un protocolo de purificación del toxoide pertúsico que garantice un grado de pureza alto y un rendimiento de más de 80% a partir de cultivos de un mutante de B. pertussis obtenido en el Centro Nacional de Investigaciones Científicas (CNIC). Métodos: para purificar el toxoide se usó una columna de intercambio catiónico Fractogel EMD SO-3 y se eluyó incrementando el pH y la concentración salina por pasos hasta obtener los resultados esperados. La determinación de proteínas totales se realizó mediante el micrométodo linealizado de Bradfor10 usando como patrón albúmina de suero bovina (BSA). Resultados: se obtuvo 3,28 mg de toxoide por cada litro de cultivo con un rendimiento de 86,7% y un grado de pureza de 96,7%. Conclusiones: el procedimiento de purificación descrito permite obtener el toxoide pertúsico con un rendimiento mayor que el 80% y alto grado de pureza, lo que garantiza la calidad requerida para su caracterización bioquímica e inmunológica.


Introduction: pertussis toxoid is a widely used in acellular vaccines against pertussis protein. Develop a protocol for purification with few steps and with parameters suitable purification is very important for chemical characterization and assessment of adverse effects that can lead to inoculate mainly mice. Purification protocols described in scientific literature involve several purification steps and low yields. Objective: to develop a protocol for purification of pertussis toxoid to ensure a high degree of purity and a yield of over 80% from cultures of a mutant of B. pertussis obtained at the National Center for Scientific Research (CNIC). Methods: for purifying the toxoid cation a Fractogel EMD SO-3 cationic interchange column was used by increasing the pH and salt concentration to obtain the expected results. The total protein determination was performed using linearized macromethod of Bradfor10 using as bovine serum albumin standard (BSA). Results: 3.28 mg toxoid was obtained per liter of culture with a yield higher than 86.7% and a purity of 96.7%. Conclusions: the purification process described allows for the pertussis toxoid with a higher yield than 80% and high purity, ensuring the quality required for biochemical and immunological characterization.

ABSTRACT

Se realizó un estudio descriptivo y transversal de 51 pacientes con neoplasia renal, atendidos en el Servicio de Urología del Hospital Pediátrico Sur Docente de Santiago de Cuba, desde enero de 1999 hasta diciembre de 2013, a fin de caracterizarles desde el punto de vista clinicoterapéutico. En la serie prevalecieron el sexo femenino, el grupo etario de 0-4 años, la masa tumoral abdominal como manifestación clínica principal, el nefroblastoma como tipo histológico más común, así como el estadio III de la enfermedad. Los pacientes fueron tratados según el protocolo oncoespecífico cubano, con predominio de la cirugía, conjuntamente con la quimioterapia y la radioterapia. La evolución de los afectados fue buena y la proporción de fallecidos en estadios clínicos iniciales resultó mínima.


A descriptive and cross sectional study of 51 patients with renal neoplasia, assisted in the Urology Service of the Southern Teaching Pediatric Hospital in Santiago de Cuba was carried out from January, 1999 to December, 2013, in order to characterize them from the clinical and therapeutic points of view. Female sex, the age group 0-4 years, the tumoral abdominal mass as main clinical manifestation, the nephroblastoma as the most common histological type prevailed in the series, as well as the stage III of the disease. Patients were treated according to the oncoespecific Cuban protocol, with prevalence of surgery, together with the chemotherapy and the radiotherapy. The clinical course of those affected was good and the proportion of dead patients in early clinical stages was minimal.

To review the epidemiology of nosocomial extended spectrum β-lactamase-producing Enterobacteriaceae in Latin America, a systematic search of the biomedical literature (PubMed) was performed for articles published since 2005. Rates of nosocomial infections caused by extended spectrum β-lactamase-producing Enterobacteriaceae in Latin America have increased since 2005. Up to 32% of Escherichia coli and up to 58% of Klebsiella pneumoniae isolates are extended spectrum β-lactamase-positive, rates that are higher than in other world regions. From a region-wide perspective, 11-25% of E. coli isolates and 45-53% of K. pneumoniae isolates were nonsusceptible to third-generation cephalosporins. At the country level, there was a wide range in Enterobacteriaceae resistance rates to third-generation cephalosporins, with especially high rates of resistance to E. coli in Guatemala, Honduras, and Mexico, and high resistance rates to Klebsiella spp. in Argentina, Brazil, Chile, Guatemala, Honduras, and Paraguay. Susceptibility of extended spectrum β-lactamase-producing Enterobacteriaceae to cefepime, fluoroquinolones, ampicillin/sulbactam, aminoglycosides, and piperacillin/tazobactam has also been compromised, leaving the carbapenems, tigecycline, and colistin as the only antibiotics with >90% susceptibility rates. There is a steady increase in the prevalence and types of extended spectrum β-lactamases produced by Enterobacteriaceae isolates in Latin American hospitals (particularly CTX-Ms), suggesting endemic conditions overlaid by clonal outbreaks. Appropriate treatment decisions and infection control strategies informed by surveillance of regional and local susceptibilities and mechanisms of resistance are required to mitigate this major public health concern.

ABSTRACT

El sobrepeso y la obesidad representan serios problemas de salud pública. Se realizó un estudio descriptivo y correlacional de variables asociadas al sobrepeso y la obesidad en preadolescentes mexicanos escolarizados en México DF. Se valoró la Autoestima general (LAWSEQ, por sus siglas en inglés), Autoestima corporal (BES, por sus siglas en inglés), Actitudes hacia la alimentación (ChEAT, por sus siglas en iglés) e Imagen corporal (BIA, por sus siglas en inglés) y comparaciones por género y edad. La muestra fue de 600 alumnos, niños y niñas de entre nueve y doce años. Se encontró una baja autoestima general (más baja en las niñas y en los participantes de mayor edad) y corporal y una buena actitud hacia la alimentación. Los participantes se percibieron y creyeron ser vistos con sobrepeso y obesidad, pero a futuro e idealmente desearon figuras más delgadas. Se propone que los programas de prevención contemplen educación para la salud, actividad física, control de aspectos genéticos, actitudes funcionales (tendientes a una buena autoestima y actitudes positivas hacia la comida sana), e involucramiento de la familia y el entorno social.


Overweight and obesity are serious public health problems. A descriptive and correlational study of variables associated with overweight and obesity of Mexican school preadolescents in Mexico DF was performed. General self-esteem (LAWSEQ, for its English acronym), body esteem (BES, for its English acronym), Attitudes towards food (ChEAT, for its English acronym) and body image (BIA, for its English acronym) were assessed and comparisons by gender and age were drawn. The sample was formed by 600 students, boys and girls aged between nine and twelve. A general low self-esteem and body esteem (lower in girls and in older participants) as well as a good attitude toward eating were found. Participants perceived themselves and believed they were seen as overweight and obese but ideally they wanted thinner figures in the future. Prevention programs are proposed to include health education, physical activity, genetic control, functional attitudes (geared toward generating good self-esteem and positive attitudes toward healthy eating), as well as involvement of the family and social environment.

Os participantes se perceberam e acreditaram ser vistos com sobrepeso e obesidade, mas a futuro e idealmente desejaram figuras mais magras. Propõe-se que os programas de prevenção contemplem educação para a saúde, atividade física, controle de aspectos genéticos, atitudes funcionais (tendentes a uma boa autoestima e atitudes positivas com respeito à comida saudável), e envolvimento da família e do entorno social.

OBJECTIVE: To establish a resistance (R) surveillance program monitoring antimicrobial susceptibility patterns in Latin America (LATAM; Argentina [ARG], Brazil [BRA], Chile, Colombia [CBA], Costa Rica, Ecuador [ECU], Guatemala [GUA], Mexico [MEX], Panama [PAN], Peru, and Venezuela [VEN]). METHODS: In 2011, 4979 organisms were collected from 11 nations (20 laboratories) for susceptibility testing in a central laboratory design. Antimicrobials were tested by CLSI methods and results interpreted by CLSI and EUCAST breakpoints. Most common Gram-positive (Staphylococcus aureus [SA, 921], other staphylococci [CoNS; 299], enterococci [218], Streptococcus pneumoniae [SPN; 182], β-haemolytic streptococci [115]) and Gram-negative (E. coli [EC; 644], Klebsiella spp. [KSP; 517], Enterobacters [272], Pseudomonas aeruginosa [PSA; 586], Acinetobacters [ACB; 494]) pathogens were analyzed against linezolid (LZD), vancomycin (VAN), tigecycline (TIG), colistin (COL), cefoperazone/sulbactam (C/S), and amikacin (AMK). RESULTS: MRSA rates varied from 29% (CBA, BRA) to 79% (Peru); but LZD (MIC90, 2 mg/L), TIG (MIC90, 0.12mg/L) and VAN (MIC90, 1mg/L) covered all strains. Enterococci showed a 14% VRE rate, highest in BRA and MEX; all inhibited by TIG and daptomycin, but not LZD (three non-susceptible with G2576T mutations or cfr). Penicillin-R among SPN and viridans streptococci was 51.6 and 41.1%, respectively. LZD overall R against Gram-positives was 0.3%. High ESBL rates were observed in EC (54-71%) and KSP (>50%) from GUA, MEX and Peru, and six nations, respectively. Carbapenem-R in KSP was 9%, highest rates associated with KPC in BRA, CBA, ECU, PAN and VEN; also a NDM-1 in KSP from CBA. AMK, TIG, C/S and carbapenems were the broadest-spectrum agents tested against Enterobacteriaceae. Only COL inhibited >90% of PSA; COL and TIG (<2 mg/L) covered >85% of ACB. CONCLUSIONS: LATAM nations demonstrated variable levels of antimicrobial R especially among Enterobacteriaceae (β-lactamase-mediated), PSA and ACB. MRSA (48%), VRE (14%) and multidrug-R SPN were also regional therapeutic challenges.

ABSTRACT

Se presenta el caso clínico de un adolescente de 15 años de edad, con antecedente de piodermitis, quien fue remitido al Hospital Docente Pediátrico Sur de Santiago de Cuba por presentar fiebre, dolor lumboabdominal del lado derecho y afectación del estado general. Desde los puntos de vista clínico e imagenológico se diagnosticó un tumor en el riñón derecho, lo cual obligó a realizarle una nefrectomía de dicho riñón. Los hallazgos anatomopatológicos confirmaron la presencia de pielonefritis xantogranulomatosa.


The case report of a 15 year-old adolescent is presented, with a history of pyodermitis who was referred to the Southern Children Teaching Hospital from Santiago de Cuba for presenting with fever, right lumbar abdominal pain and a compromised general status. From the clinical and imaging points of view a tumor was diagnosed in the right kidney, which forced to perform a nephrectomy of this kidney. The pathological findings confirmed the presence of xanthogranulomatous pyelonephritis.

Durante el embarazo aumentan el metabolismo basal, el consumo de O2, la frecuencia cardíaca, el volumen sistólico, el volumen minuto y la volemia y disminuyen la tensión arterial y la resistencia periférica. Diferentes estudios han demostrado que durante este período la posibilidad de que ocurra una arritmia cardíaca o se produzca la exacerbación de una arritmia preexistente es mayor. No obstante, en su enorme mayoría carecen de importancia pronóstica tanto para la madre como para el feto. La taquicardia ventricular del tracto de salida del ventrículo derecho es una arritmia poco frecuente y su aparición se ha correlacionado con el aumento del tono adrenérgico. Se presentan los casos de dos pacientes que mostraron en el curso de la gestación reiterados episodios de taquicardia ventricular del tracto de salida del ventrículo derecho. Se analiza la asociación del embarazo con la ocurrencia de trastornos del ritmo cardíaco.


During pregnancy, there is an increase in metabolism, oxygen consumption, heart rate, stroke volume, cardiac output, blood volume and a decrease in blood pressure and peripheral resistance. Studies have shown that during this period the occurrence of cardiac arrhythmias is not uncommon. Fortunately, malignant arrhythmias are rare. Herein we report two young patients who presented with symptomatic right ventricular outflow tachycardia during pregnancy that required antiarrhythmic therapy. Possible pathophysiologic mechanisms are discussed.

El miocardio no compactado es una rara miocardiopatía congénita caracterizada por la presencia de múltiples y prominentes trabeculaciones profundas en la pared ventricular que definen hendiduras comunicantes con el compartimiento ventricular principal. Es una entidad de baja incidencia y prevalencia que se diagnostica mediante estudios de imágenes como el ecocardiograma Doppler (ED), la tomografía computarizada multicorte (TCM) y la resonancia magnética cardíaca (RMC). Puede ser asintomática o manifestarse mediante arritmias, insuficiencia cardíaca o eventos tromboembólicos. Se presenta el caso de un hombre de 33 años, asintomático, que durante la práctica deportiva sufre una conmoción cardíaca (commotio cordis) que le produce un paro cardiorrespiratorio. El electrocardiograma mostró un ritmo de aleteo ventricular que requirió cardiodesfibrilación eléctrica. En un ED efectuado inicialmente no se observaron anormalidades significativas, pero otro ED, una TCM y una RMN obtenidos luego del alta, certificaron el hallazgo de miocardio no compactado aislado, descartando coronariopatía. Recibió tratamiento beta bloqueante y antiagregante y se discutió la colocación del cardiodesfibrilador implantable. Se plantea la fisiopatología de la asociación de estas dos infrecuentes y potencialmente letales afecciones.


Non compact of the left ventricular myocardium is a rare congenital cardiomyopathy characterized by the presence of multiple and prominent deep trabeculations in the ventricular wall, that define recesses communicated with the main ventricular chamber. This is a condition with low incidence and prevalence, diagnosed through imaging techniques such as Doppler echocardiogram (DE), multi-slice computed tomography (MSCT) or magnetic resonance imaging (MRI). Clinically, it may be asymptomatic or manifested by cardiac arrhythmias, heart failure or thromboembolism. This is a report on a 33 year old asymptomatic man who suffered a blow on his chest (commotio cordis) during a sports competition that produced a cardiac arrest. The electrocardiogram showed ventricular flutter that required electrical defibrillation. A DE obtained initially, did not show any significant abnormality, but another DE, a MSCT and a RMN obtained after discharge, certified isolated non-compacted myocardium, ruling out coronary artery disease. He received beta blocker and antiplatelet therapy and the placement of an automatic cardioverter defibrillator was considered. The pathophysiology of the association of these two infrequent and potentially lethal conditions is discussed.

El hipotiroidismo en el embarazo es infrecuente, pero cuando ocurre suele asociarse con complicaciones maternas y fetales. Se presenta el caso de una mujer joven sin antecedentes de enfermedad cardiovascular que consulta por ortopnea, dolor torácico y edema de miembros inferiores. Los exámenes pusieron en evidencia la existencia de insuficiencia cardíaca, hipotiroidismo, síndrome nefrótico e insuficiencia renal. El eco-Doppler mostró dilatación de las cuatro cavidades cardíacas con deterioro grave de la función sistólica. El tratamiento con levotiroxina por vía intravenosa mejoró el cuadro clínico y los parámetros de laboratorio. Se analizan los efectos de la hormona tiroidea sobre el aparato cardiovascular y se comentan los mecanismos fisiopatológicos de la insuficiencia cardíaca en el embarazo.


Hypothyroidism during pregnancy is infrequent, but its presence is associated with maternal and fetal complications. We present the case of a young pregnant woman with no previous history of cardiovascular disease, who consulted for orthopnea, chest pain and edema in both legs. Laboratory tests demonstrated a hypothyroid condition and a nephrotic syndrome with renal failure. The echo-Doppler exam showed a four chamber dilatation with systolic dysfunction. Treatment with intravenous levothyroxine improved her medical condition. We analyze the effects of thyroid hormone on the heart and vascular system and discuss the pathophysiologic mechanisms of heart failure during pregnancy.

OBJECTIVES: This analysis of the Study for Monitoring Antimicrobial Resistance Trends (SMART) evaluated the susceptibility patterns of Enterobacteriaceae in Latin America in 2008, with emphasis on susceptibility trends of E. coli and K. pneumoniae. METHODS: Clinical isolates were recovered from intra-abdominal infections (IAI) from 23 centers in 10 Latin American countries. Isolates were sent to a central laboratory for confirmation of identification, antimicrobial susceptibility and ESBL testing, following the Clinical Laboratory Standards Institute (CLSI) guidelines. RESULTS: Of 1,003 Gram-negative bacilli collected from intra-abdominal infections, E. coli and K. pneumoniae were the most commonly isolated organisms, and 26.8 percent of E. coli and 37.7 percent of K. pneumoniae were ESBL positive. Ertapenem and imipenem were the most consistently active agents tested; 99 percent of ESBLpositive E. coli isolates were susceptible to ertapenem and 100 percent to imipenem as well, and 91 percent of ESBL-positive K. pneumoniae were susceptible to ertapenem and 98 percent to imipenem. Quinolones and cephalosporins were less active, achieving 1.5 percent to 76 percent inhibition against ESBL-producing E. coli and 3.5 percent to 61 percent inhibition against K. pneumoniae. CONCLUSIONS: Local and unit-specific surveillance data is particularly important for selection of empiric therapy and in community-acquired infections as they can help the clinician with antibiotic selection by providing guidance regarding the likely pathogens and their resistance profiles. Our data also confirm the increasing frequency with which ESBL-producing organisms are found in the community setting, with 31.4 percent of communityacquired and 24.9 percent of hospital-acquired infections found to produce ESBLs. Imipenem and ertapenem are the most active agents tested for ESBL-positive E. coli and K. pneumoniae.

ABSTRACT

Surveillance systems monitoring the spread and divergence of methicillin-resistant Staphylococcus aureus (MRSA) strains are critical if preventive and therapeutic measures targeting MRSA infection are to be employed optimally. Surveillance provides information on the spread of MRSA, on the emergence of new strains within hospitals and communities, on the antibiotic resistance profile and virulence of strains, and on the risk factors associated with infection. These data help clinicians to provide appropriate empiric treatment of infections circulating in their region, leading to improved patient outcomes. While information on MRSA epidemiology in Latin America is growing, significant gaps exist in the available data, especially in local areas where fewer resources are available for characterizing and reporting MRSA strains. Here, we describe current knowledge of healthcare- and community-associated MRSA epidemiology in the region, and provide recommendations for future development of surveillance systems with a view to providing robust data at regional, national and local levels.

ABSTRACT

Strategies to monitor and control the spread of methicillin-resistant Staphylococcus aureus (MRSA) infections are dependent on accurate and timely diagnosis of MRSA in both hospital and community settings. In Latin America, significant diversity in diagnostic and susceptibility testing procedures exists at the regional, national and local levels. Various tests for S. aureus and MRSA are available in clinical settings, but the application of these techniques differs between and within countries, and quality control measures are not uniformly applied to verify diagnoses. To optimize the diagnosis of MRSA infections across Latin America, a more consistent approach is required. This may include: adoption and appropriate adaption of specific guidelines for MRSA testing, depending on local resources; establishment of a coordinated system for quality control; regional access to central reference facilities; education of medical and healthcare professionals in best practices; and development of systems to evaluate the implementation of guidelines and best practices.
